Below, please list extra-curricular school activities and community volunteer groups in which you participated to a significant degree during your time in high school. Be sure to list your role, the approximate time commitment for each activity, and your grade(s) at the time of participation, if applicable. Be sure to list only one activity/honor per line.
All sports, clubs or activities you participated in that are affiliated with your school
Example: Marching Band, 10 hours/month. Sept.-Dec., 9th 10th 11th & 12th grades
Example: National Honor Society, 4 hours/month. Sept.-Dec., 11th & 12th grades
All charitable organizations or activities that you valunteered at that are not affiliated with your school. This includes any unpaid volunteer work done in the community or to support the mission of a charitable, civic, or community organization.
Example: Youth Basketball Coach, 12 hours/month Jan. & Feb., 10th & 11th grades
Example: Red Cross Blood Drive, 6 hours once, March, 9th grade
Example: Honor Roll, 1st Semester, 11th Grade
Attach a personal statement (maximum one typed page unless otherwise noted) that will give the Selection Committee
insight into you as an individual. Include qualities related to your achievements, character, and
potential that would make your selection as a scholarship recipient a worthwhile investment. You may also
discuss why you are pursuing a certain major or career. Please take note that some scholarships
require a specific theme for the personal statement which will be noted in the individual scholarship descriptions.
Attach an official copy of your latest academic transcript for each application you are submitting.
A one page copy of your FAFSA Student Aid Report listing your Expected Family Contribution (EFC #) is required for scholarships with financial need as a selection criterion. Please note that in addition to the FAFSA information some scholarships may require a summary sheet created by you, indicating your need for financial assistance.
