Narrative

  • Below, please list extra-curricular school activities and community volunteer groups in which you participated to a significant degree during your time in high school. Be sure to list your role, the approximate time commitment for each activity, and your grade(s) at the time of participation, if applicable. Be sure to list only one activity/honor per line.

  • Attach a personal statement (maximum one typed page unless otherwise noted) that will give the Selection Committee insight into you as an individual. Include qualities related to your achievements, character, and potential that would make your selection as a scholarship recipient a worthwhile investment. You may also discuss why you are pursuing a certain major or career. Please take note that some scholarships require a specific theme for the personal statement which will be noted in the individual scholarship descriptions.

  • Attach an official copy of your latest academic transcript for each application you are submitting.

  • A one page copy of your FAFSA Student Aid Report listing your Expected Family Contribution (EFC #) is required for scholarships with financial need as a selection criterion. Please note that in addition to the FAFSA information some scholarships may require a summary sheet created by you, indicating your need for financial assistance.

    Please make sure all of the following steps are completed before submitting application materials to your high school counselor.

    • Complete this application process and print 3 copies for submission on the next page. Please note that some scholarships require a special application which is noted in the individual scholarship criteria/requirements. Special applications can be found online at www.c3f.org unless otherwise noted.
    • Be sure that the name of the scholarship you are applying for is at the top of each application form.
    • Complete all portions of the application as required to meet the criteria of the specific scholarship. If any required documents are missing your application will not be considered.
    • Include a copy of your latest high school transcript (and college transcript if applicable).
    • Include a personal statement, not to exceed one typed page unless otherwise noted. Note that some scholarships require a specific theme for the personal statement which would be noted in the individual scholarship criteria/requirements.
    • If required (see individual scholarship criteria/requirements), include a copy of the page of your FAFSA Student Aid Report that lists your Expected Family Contribution (EFC #). For more information and to obtain your FAFSA report, visit www.fafsa.ed.gov. Note that instead of the FAFSA information, some scholarships may require a summary sheet created by you, indicating your need for financial assistance.
    • Sign the application form.
    • Include the original and two (2) photocopies of all application materials for each scholarship for which you are applying. Do not put in binders/notebooks and be sure all three (3) packets are identical when submitted.
    • Deliver all completed materials to your high school counselor's office by Thursday, March 1, 2018, unless otherwise indicated.

    Important Note

    Please be careful to submit accurate and complete applications. Applications that are late, do not meet the criteria, are missing a required document, or do not have one original and two (2) copies of the appropriate materials will not be considered. Faxed or e-mailed applications will not be accepted.
